To undertake and support high quality, outcome-led, individual carer conversations.

Work with carers and colleagues to improve outcomes for carers in a way that ensures early identification and promotes the benefits of supporting carers to maintain their caring role.

To provide information and advice to carers around welfare and support available.

To signpost and assist carers to make connections in appropriate pathways - Citizens Advice, Carer Support Groups etc.

To link with other professionals to ensure a responsive approach to meeting the needs of carers in an outcome focused way., All employees are required to comply with their individual and organisational responsibilities under the Data Protection Act, the Information Security Policy and relevant supporting operational policies. Any matters of a confidential nature must not be disclosed or passed to any unauthorised persons or third party under any circumstance either during or after employment except in the proper course of your employment or as required by law, Merthyr Tydfil County Borough Council or both. Ability to speak Welsh is desirable.
