Full Job Description

  • To undertake and support high quality, outcome-led, individual carer conversations.


  • To support Merthyr Tydfil's contribution to the Cwm Taf Carer Strategy.


  • Work with carers and colleagues to improve outcomes for carers in a way that ensures early identification and promotes the benefits of supporting carers to maintain their caring role.


  • To provide information and advice to carers around welfare and support available.


  • To signpost and assist carers to make connections in appropriate pathways - Citizens Advice, Carer Support Groups etc.


  • To link with other professionals to ensure a responsive approach to meeting the needs of carers in an outcome focused way., All employees are required to comply with their individual and organisational responsibilities under the Data Protection Act, the Information Security Policy and relevant supporting operational policies.     Ability to speak Welsh is desirable.
