Full Job Description

Wimborne and Ferndown PCN are looking for a keen individual to join our PCN team to help enhance services for our patients. Our PCN includes the following practices: The Quarter Jack Surgery / The Old Dispensary, Walford Mill Medical Centre, Pennys Hill Practice and Orchid House Surgery., The role of the Ageing Well Coordinator is to support the wider PCN team by administratively supporting the PCN Management team to deliver on elements of the PCN DES and other initiatives as directed by NHS England. The role will also support Clinicians within the team to ensure smooth running of their clinics and bookings., The role is a mixture of administration and care coordination, supporting teams in delivering the key performance indicators of the Network DES and other key network service areas. Additionally during the Covid-19 Vaccination Programme, the Network Care Coordinator will support administratively the requirements of the vaccination hub.

The role will include:

  • Managing clinics to ensure each clinician is fully utilised

  • Work alongside our Digital Transformation Lead to develop digital initiatives within the PCN.

  • Support the PCN Management in delivering services required within the PCN.

  • To take minutes during team meetings.

  • Booking and cancelling appointments.

  • Running reports on SystmOne and the DiiS.

  • Analyzing data.

  • Check in reception duties.

  • Contacting patients via phone.

  • Organising events for patients and for staff.

    Wimborne & Ferndown PCN provides services to 42,000 patients across 4 Practices (Quarter Jack Surgery/ Old Dispensary, Walford Mill Medical Centre, Pennys Hill Practice, Orchid House Surgery). We have adopted new roles within the PCN to support and broaden our primary care team which allows us to provide better care for our patients.


  • The PCN is headed by our committed Clinical Directors and a PCN Manager.

    In addition to the PCN, our Practices are also part of Castleman Healthcare, a non-profit company owned by 13 GP Practices in North Poole and East Dorset (3 PCNs). Castleman Healthcare offer additional services to our Patients through separate contracting arrangements, provide business and practical support to the PCNs and support with representing the PCNs voice within the wider healthcare system.
