Full Job Description

Youth Works, on behalf of the REACH partnership of Youth Counselling services in Northamptonshire has been commissioned by NHS Northamptonshire to deliver a service to support young people making the transition from Child and Adolescent Mental Health Services to Adult Mental Health Services.

This can be a difficult and challenging time for many young people. Support is offered to young people via a referral system. The RE:Start Service introduces young adults to the comprehensive crisis pathway available across Northamptonshire, and ensures co-produced, personalised Safety Plans are created for each young adult, to help avoid unnecessary attendance at emergency departments.

The Service uses a 'Personal Assistant (PA)' approach to ensure a flexible, holistic approach to care. It has the capacity to work with CYP from 16 years - building trust and gaining a robust understanding of needs - and to remain involved with young adults up to the age of 25 to ensure a gradual transition into adulthood.

The ethos and culture of the service is solution-focused, strengths-based, personalised, flexible and is able to work with a complex set of needs, regardless of diagnosis.

There are two teams of support workers, one in the north of the county and one in the west.

We are looking to appoint a Health Inequalities / Accessibility Lead within our RE:Start team to link with groups across the county where health inequalities have been identified to improve accessibility for all young people who may benefit from the RE:Start project. They will be identifying barriers that prevent young people from using our services and then support the implementation of the actions that arise. This role will also include promoting our services in the local community.

Job Purpose

They will be identifying barriers that prevent young people from using our services and then support the implementation of the actions that arise. This role will also include promoting our services in the local community.

General Duties

  • Identify barriers to using our services across all protected characteristics / groups and then put improvements / actions in place to mitigate them.

  • Promote the RE:Start service in the local community - local events (including mapping), education establishments (including speaking at assemblies), local and public sector organisations, relevant businesses etc.

  • Identify any gaps in the services we provide ensuring they are inclusive and accessible for everyone.

  • Develop a stakeholder map and build relevant key relationships within the local community.

  • Engage proactively with systemwide forums where health inequalities are being addressed and discussed. For example Adult MH pillar and/or Enabler meetings

  • Undertake any other appropriate duties/tasks that fall within the general scope of the role.

  • Attend any other additional meetings and training as required
