Full Job Description

The post-holder will be an experienced nurse/paramedic, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patient within the general practice.

The post-holder will provide this service to our care home residents, patients in their own homes and at the surgery.

The post-holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

The post-holder will build effective working relationships with other health care professionals.

The post-holder will work within the team of GPs, nurses and health care assistants to deliverexcellent primary health care services within the Practice., To deliver a high standard of patient care as an Clinical Care Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

Together we serve a population of 50,000 patients. The expectation is that the successful candidate will collaborate with our GPs, Nurse Practitioners, Nursing Team and Pharmacists, as well as our colleagues in Community Nursing, Physiotherapy, Occupational Therapy, Social Care and at SWAST.
